Companions provide the support and supervision that will enable you or your loved one to remain independent, while getting the assistance you need to remain safe and comfortable in your own home. Services include:

Specialty
Companions are available for:

Alzheimer’s Care Companions are skilled at communicating with people diagnosed with Alzheimer’s. They are also trained in recreational activities, music and touch therapies, and all components to care for patients with Alzheimer’s.
• Hospice Care Companions are skilled at caring for terminally ill patients. They bring compassion and the ability to attend to your loved one’s needs during their final days.
Homemakers
A homemaker can help you maintain your home the way you want, ensuring a clean and safe environment. Services include:
• Errands
•
General cleaning
• Grocery shopping
• Housekeeping and services
• Laundry
• Meal planning and preparation
• Medication reminders
Personal
Care Assistant
Personal Care Assistance is available for those who are in need of assistance with personal hygiene and more. Services include:
• Assistance with ambulation
• Bathing
• Dental Hygiene Assistance
• Dressing
• Medication reminders
• Shampooing and Drying hair
• Toileting
Personal
Care Assistants respect one’s dignity, while helping to maintain
independence in the home.
Live In Care
A Live In can provide invaluable peace of mind for a family member or other caregiver while they work, shop or go on vacation, ensuring both security and socialization for their loved one. Whether it is to share a game, read, serve a meal or just listen, a trustworthy companion from A Caring Hand can be there. Services include:
• 24 hour care
• Caregiver respite
• Companionship

• Medication reminders
• Shopping and errands
Geriatric Care Management
A Geriatric Care Manager (GCM) is a resource expert who specializes in assisting with senior care issues as well as long term care arrangements for older adults and their families. Geriatric Care Management personnel are trained and experienced in the assessment, coordination, monitoring and direct delivery of services to help clients remain in the least restrictive environment. A GCM will provide a complete assessment of the client. Together, the client, family and GCM are involved in the Plan of Care. Needs may change rapidly and through close monitoring the GCM can make the necessary and immediate changes to help clients living at home remain as safe and independent as possible.
A Geriatric Care Manager will:
• Provide a comprehensive medical, physical and social needs assessment
• Develop, coordinate and modify a care plan to assist with day-to-day activities
• Arrange and monitor in-home help or services
• Act as a liaison to families at a distance
• Provide a link to community resources available to client
• Review financial, legal, or medical issues and refer to the appropriate resources
• Assist with town, state and federal applications, (e.g. Medicaid applications)
• Provide counseling and crisis intervention to address emergency situations
• Assist with alternative living arrangements when home is no longer an option
• Provide ongoing support and counseling to clients’ families
Rates
for Services:
Companions
$16.65 per hour
Homemakers
$18.75 per hour
Personal Care Assistant $20.80 per hour
Live
in Companions $220.00 per day
Please
note the $.57 cents per mile (may be subject to
change) will be charged for all client transportation.
Please
note that mileage will be charged for all client transportation.
The
following Holidays will be paid and billed at time and a half:
New
Years Day (starting New Years Eve after 5 pm)
Memorial Day
Fourth of July
Labor Day
Thanksgiving
Christmas Day (starting Christmas Eve
after 5 pm)
All
rates are determined upon a 3 hour minimum except those that have
been determined to meet individualized needs.
